Gerald "Jerry" Joseph Kozak

 

 

Phoenix - Gerald "Jerry" Joseph Kozak, age 74, of Phoenix, passed away on Wednesday, June 10, 2020 in Phoenix, Arizona. He was born on July 15, 1945 to the late Stanley and Ann (Swietek) Kozak in Buffalo, New York. On June 10, 1967 he married Francine Theresa Chlebowski in Buffalo, New York. Jerry had a distinguished career as President and Chief Executive Officer of Family and Children's Service of Niagara from 1976-1997. He then worked for Arizona Child Protective Service from 1998 until he retired in 2010. Jerry is survived by his loving spouse of 53 years, Francine; two children, David Kozak, spouse Jaime Kozak and Jennifer Lutton, spouse James Lutton; his brother Donald Kozak and sister Audrey Niewiemski; four grandchildren, Brendon Lutton, Sydney Lutton, Elizabeth Kozak, and Lillian Kozak. Arrangements for a service will be made at a later date.
